I, _______________________, as a parent / guardian of ________________________ give my permission for him/her to attend WildCard Sports, LLC. All participants agree to abide by the rules set by WildCard Sports, LLC for the health, safety, and welfare of all participants. WildCard Sports, LLC retains the right to remove any child from class or the program who exhibits behavior deemed inappropriate by the coaches or staff.
ASSUMPTION OF RISK- Participation in gymnastics and other related activities involving motion rotation, and other activity in a unique environment and as such carries with it a certain assumption of risk. The undersigned and the student elect voluntarily to enter upon said premises which are owned or under the control of or are participated in as a scheduled activity of WildCard Sports, LLC, knowing that the condition thereof and the activity involved may be or become hazardous or dangerous during the time that the student or the undersigned is upon the said premises. The undersigned and the student voluntarily assume all risks of loss, damage, or injury that may be sustained by the student and/or undersigned or any property owned by them while in, on or upon said premises described above.
RELEASE AND WAIVER- In consideration of allowing the named student to enroll in the WildCard Sports, LLC program and the use by the student of the premises and property of WildCard Sports, LLC, the undersigned, acting both or themselves and on behalf of the student, do hereby agree to release and to indemnify, hold harmless, and defend WildCard Sports, LLC, its owners, directors, officers, employees, agents, and representatives, or while in route to or from any said premises, or while at any other premises, or place when undertaking activities sponsored by or participated in by WildCard Sports, LLC, its owners, directors, officers, employees, agents, or other representatives.
MEDICAL EMERGENCY- I undersigned hereby agree to allow his/her child or ward to receive medical aid, as needed in the determination of WildCard Sports, LLC, its owners, directors, employees, agents, and other representatives
THE UNDERSIGNED hereby agree acknowledge (A) that he or she has read the foregoing, and in particular the release and waiver and the assumption of risks paragraphs. And do hereby sign the same voluntarily; and (B) that any undersigned as a parent or as a legal guardian is, in fact, the true parent or legal guardian and has the consent of the student.
